This program provides community-based health care to help eligible seniors* living in the Ottawa West catchment area who are hospitalized, or at high risk of hospitalization, successfully return, or safely remain,  in their own home.

Assisted Living Services focuses on activities of daily living that the individual finds challenging to do on their own including:

• Personal Care
• Homemaking
• Safety Checks

The program also offers 24-hour urgent response.
