This innovative program provides a variety of services to seniors living in Ottawa Community Housing (OCH) apartment buildings including:

  • Homemaking & meals
  • Transportation
  • Nursing & Allied Health visits
  • Personal Care
  • Case Management
  • Rapid Community Support Referral
  • Crisis Intervention

The program aims to reduce Emergency Room visits and hospitalization through a partnership between a Home and Community Care Case Manager and an on-site Ottawa West Community Support Outreach Coordinator who coordinate the delivery of Foot Care Clinics, Meals on Wheels, and Health Promotion activities.

The AIP program team works closely with key community partners including Ottawa Public Health, The Ottawa Community Support Coalition and many other agencies  to reduce health care access barriers and support seniors in remaining independent in their homes.
